Add photo/attachment capture to the entry sheet's photo field: tap to take a photo or pick one from the gallery, feed it through the existing PhotoIngest -> AttachmentUploadEngine pipeline (#29), and show upload-progress/failed states with remove support. Also closes a latent clobber bug where a completed upload's embed could be silently overwritten by the next unrelated field edit.
